Although raw milk is not processed in a technological sense, it is subject to biological processing. The development of the mammary gland during juvenile life and, especially, during gestation is followed by lactogenesis the initiation of secretion , secretion itself, storage of the product prior to removal, and then milk ejection. During the lactation secretory cells are subject to developmental remodeling especially just after parturition , repeated mechanical disruption during alveolar filling and emptying, potential attack by pathogenic bacteria, and finally, at the end of the lactation, regression and involution.
All of these processes have been extensively research and reviewed before, and rather than simply repeating that exercise I shall try to identify those elements of lactation physiology that are most relevant to safe consumption of raw milk and also to be prospective rather than retrospective wherever possible. Fortunately the older literature, most of which is still valid, was excellently reviewed relatively recently in a series of papers published in the Journal of Mammary Gland Biology and Neoplasia hereafter JMGBN; Neville, For the purposes of this chapter, the different physiological processes that together constitute lactation are shown in Fig.
The reason for mentioning this is to highlight the multitasking nature of lactation, for it is quite evident that milk provides a triad of nutrition, protection, and hydration. The relative importance of these functions will vary to some extent from species to species, a simple example being the transfer of passive immunity in colostrum, which is much less essential in humans than in cows because of transplacental transfer to the human fetus prepartum.

Another important property of milk is the way in which the nutrient is delivered, calcium being a case in point; nanoclusters of calcium phosphate are present as substructures in casein micelles at concentrations well above their solubility Lenton et al. Occasionally a particular milk might have a specific or even unique characteristic. For instance, there is growing evidence that camel milk can deliver significant quantities of biologically available insulin Meena et al.
Having decided to secrete milk and agreed on what to put into it, for how long should lactation continue? This introduces another concept that is important for any discussion of raw milk. The biologically intended consumer of milk is the neonate, but in this book we are mainly considering older consumers.
The nutritional requirements of the human neonate are normally well met by breastmilk, although there is sometimes debate about the detail Knight, and Raw Milk Physiology 5 increasing recognition that human lactation can fail Marasco, The optimal requirements of older consumers will be very different and dependent on many lifestyle-related factors, especially other diet and exercise.
There has long been an epidemiologically based belief that consumption of milk, and especially milk fat, is detrimental to health, in particular cardiovascular health. More recent reexamination of the evidence has shown this to be untrue Thorning et al. At birth and well into juvenile life, the gland consists of relatively isolated ductular epithelial tissue lying in a fat pad. The mammary epithelium will not grow outside of that fat pad, and starting in early life there is clear evidence of regulatory interaction between the mesenchymal elements of the fat pad and the extracellular matrix that is formed from it and the epithelial cells of the ducts Kratochwil, ; Faulkin and Deome, Later, however, an excess of adipose tissue has local inhibitory effects on the growth of the duct system.
This has been demonstrated in the prepubertal heifer where it is proposed to be due to disturbance of the growth hormone GH : insulinlike growth factor axis Sejrsen et al. Whether this growth inhibition fully explains the subsequent reduction in milk yield observed in overfed heifers is open to debate; the udder develops progressively but slowly over a long period of time and it is not always possible to discount age-related differences in animals that grow at different rates Daniels et al.
Nevertheless, there is clear evidence in obese mice of an inhibitory effect of excess adipose deposition in the mammary fat pad on both the development and subsequent function of the secretory tissue Flint et al. Given the growing concern surrounding lactation inadequacy in obese women reviewed by Nommsen-Rivers, this is an area deserving of further investigation.
This pattern of growth was first quantified for ruminant dairy species in goats Fowler et al. Classical postwar studies of the endocrine control of mammogenesis were included in the special issue of JMGBN Forsyth and Neville, Later it became apparent that placental lactogen, a prolactin-like hormone produced by the fetoplacental unit of many species, could substitute for prolactin Hayden et al.
Berryhill et al. Ultimately, for mammary growth to occur there must be proliferation of epithelial cells. Curiously, estrogen is a mammogen, but not apparently a mitogen; in vitro studies have failed to establish any direct stimulation of mammary cell proliferation by estrogen, and instead it is believed that the action is indirect via locally produced IGF1, which appears to be the major mammary mitogen in cattle.
Numerous other factors such as photoperiod Andrade et al. Photoperiod may well act through prolactin and inflammation through other local growth factors such as colonystimulating factor CSF from macrophages the action of CLA appears to be linked to an inflammatory response.
However, showing that a particular factor can affect mammary cells is not necessarily evidence of an overt action in the pregnant animal, and it is interesting to note that the photoperiod effect referred to here was observed in sheep stimulated into lactation with exogenous steroids, whereas the CSF research was done in mice and targeted toward better understanding of breast cancer.
For dairy species, research interest in gestational mammogenesis has waned in the last decade the same is certainly not true of rodents and other breast cancer model species.
This probably results from the almost universal Raw Milk Physiology 7 success of mammogenesis in dairy species; there is little reason for thinking that it can be improved.
The mitogenic activity of IGF1 has already been mentioned. However, attempts to stimulate mammogenesis in sheep using GH have been unsuccessful Stelwagen et al. In this case the mammary glands are small and underdeveloped at parturition. The joey then attaches permanently to one teat, and this gland and no other starts to secrete milk but also begins a phase of major proliferation Lincoln and Renfree, This concurrent proliferation and secretion represents an extreme situation, and its unilateral nature emphasizes the importance of local regulatory mechanisms.
We shall return to this when considering regulation of secretion and bioactive factors in milk. In the majority of species there may be a small amount of further mammary proliferation during early lactation, but this will be small in comparison to gestation.
The primary developmental characteristic of the lactating udder of dairy animals is gradual cell loss through apoptosis beginning at around peak lactation and accounting for the characteristic decline in milk yield as lactation advances. This has relevance to the duration of the lactation, which will be discussed later. It will be evident that the development of the gland prior to the start of lactation is essential and will have major impact on the amount of milk produced.
Whether there is any significant effect on milk composition is doubtful, unless there are major deficiencies in the structural integrity of the developing epithelium. This will become evident as we start to consider the secretory process.
Secretory cells have proliferated during pregnancy, so now we need to consider these cells and their secretory activity in more detail. Fleet et al. The processes and their control are completely different, but interrelated. The problem is one of definition; colostrum is the first milk appearing after parturition, but there is good evidence to show that its secretion starts well before that, and it is then stored within the gland for days or weeks.
